Systemic Lupus Erythematosus with Purtscher-Like Retinopathy as the First Symptom

Purpose: To report a case of purtscher-like retinopathy as the primary manifestation in patient with systemic lupus erythematosus (SLE).Case summary: A 44-year-old healthy female presented decreased visual acuity both eyes. On fundus examination, multiple cotton wool spots and retinal hemorrhages on posterior pole were observed. fluorescein angiography, nonperfusion vascular leakage observed at optical coherence tomography, there increased hyperreflectivity nerve fiber layer ganglion cell layer, intraretinal fluid, subretinal fluid. serological tests including thrombophilia anti-Ro antibodies, anti-dsDNA anti-Sm antibodies positive. She was diagnosed SLE. After 1 month steroid hydroxychloroquine treatment, fluid resolved, but not recovered.Conclusions: In patients without underlying disease, it should be considered that may Purtshcer-like appear an autoimmune disease such
